COVID19 Travel/Return Form If you have traveled or a returning from a Level 2 or 3 location or a COVID-19 hotspot, the CDC recommends individuals to self-quarantine for 14 days. Buena Vista ISD has established the same recommendation for travel as the CDC guidelines. Currently, Level 2 and 3 countries include China, Iran, Japan, Italy, and South Korea. Please keep in mind, travel restrictions could change if locations are added to the risk assessment levels by the CDC. Please complete the form below if you are traveling to a Level 2 or 3 affected area. The confidentiality of your submissions is important. Your information will be kept secure at administrative offices. The Texas Education Agency is working to determine procedures to address this public health situation and absences. Buena Vista ISD will work with families on a case-by-case basis.
