Incident Reporting Form

This form is for the submission of the reporting of an alleged incident of Bullying, Harassment, Intimidation or any other concern. This form can be submitted anonymously; however the reporting of an incident does require the name of a victim and of the aggressor, if known. Once submitted, this form will be sent to James R. Webster, Director of Student Services. Each report will be reviewed and sent to the appropriate campus for investigation. Sheldon ISD encourages parents to contact their child's campus administration directly when any situation of concern arises. If the incident creates an immediate concern for student or staff safety, contact your child's campus immediately or call 911.
