Ector County ISD Student-Parent Athletic Packet

To enroll students in any ECISD Athletic activity a parent must: 1. Complete the following online form 2. Print out the Medical History and Signature pages 3. Have a physical completed on the student and the Medical form signed by a Doctor 4. Return the signed Authorized Signature page, Medical Form, and a copy of insurance card to the appropriate campus athletic staff This must be done prior to participating in any ECISD Athletic activity (even scheduled practices).
